§ 103560 Death Certificate Short Form

A short form of certification of death registration, including only identification information and excluding the medical statement of the cause of death, may be issued by the State Registrar, county recorder, or any local registrar, upon forms prescribed and furnished by the State Registrar. (Added by Stats. 1995, Ch. 415, Sec. 4. Effective January 1, 1996.)
