§ 1654 Licensee Name Change Notification

Key Takeaways

  • •If you have a license (like for work) and you legally change your name, you must tell the board that gave you the license within 10 days.
  • •The board will then write down your old name next to your new name in their records.
  • •This rule is to keep track of who has a license, even if their name changes.

Example

A nurse named Sarah gets married and legally changes her last name to Johnson.

Sarah must tell the nursing board about her new name within 10 days. The board will then note her old name (Sarah Smith) next to her new name (Sarah Johnson) in their records.
